The following paperwork must be completed in its entirety before PMSR Network is able to accept and process the tissue. The physician may also require his/her own paperwork to be completed prior to performing the procedure. Terminology
Next of kin (NOK) – if the gentleman is legally married, the NOK is his spouse. If he is unmarried (regardless of duration of relationship), the NOK are his parents.
Decedant – deceased person.
